Alliance Child & Family Solutions, ACFS Texas, partners with home health agencies to provide comprehensive social work assessments. Our evaluations are conducted in-home with you and your family members to determine what needs you may have, connect you to resources in the community, answer questions about insurance qualifications, and provide a care strategy to help with your long-term planning.
Our assessments can include the following:
- Reviewing in-home care options and providers
- Requesting durable medical equipment to help with your Activities of Daily Living (ADLs)
- Evaluating financial resources to lower your out-of-pocket costs
- Applying for medication assistance programs
- Finding in-network physicians or specialists
- Obtaining handicap placard applications
- Coordinating care with your physician or treatment team
- Enrolling with an emergency response system to prevent falls
- Filling out applications or documents related to insurance
- Reviewing and completing advanced directives for your care

All of our team members are licensed medical social workers who have completed extensive training to provide you with the highest quality of care. Our home health assessment services are provided on either a private pay basis or can be filed with your insurance if conducted through your home health care agency.
